New Patients

Each new patient will be asked to complete a Patient Health Questionnaire, Financial agreement and Consent for Medical Information form. Please download these forms from our website and be sure to bring the completed forms with you to your appointment. Please arrive 15 minutes before your scheduled appointment to allow time for check in.

Please bring your current insurance cards (including ProviderOne

Card and/or Healthy Options card) along with the required Co-payment as required by your insurance company.


We accept a wide variety of insurances including certain Medicaid plans for our OB patients only. We are constantly reviewing new insurances to be added to our list of insurances that we accept. When you call to schedule your appointment please have your insurance card(s) ready to verify that we accept your insurance(s).

Please bring current insurance cards to your appointment so that your claims can be submitted to the correct insurance companies for processing and payment. If your insurance is Medicaid you will be asked to provide your Provider One and/or Healthy Options card at each visit.

Payment Policy

For patients with insurances, all co-payments or co-insurances or deductibles must be paid at the time of service. This arrangement is part of your contract with your insurance company.

For your convenience, we accept Master Card, Visa, debit card, or personal check.

Please keep in mind that your insurance benefit is a contract between you and your insurance company. Please be aware that some and perhaps all of the services you receive may be non-covered as per your insurance policy.

Appointment cancellations / Late Arrivals

As a courtesy to other patients and the physician, we request that you to give us at least 24 hour advance notice for cancellations of any appointments so that your appointment can be provided to patients who need to be seen by the physician.

We understand that there may be a time that your appointment has “slipped your mind.” In this case you will be sent a letter informing you the importance of keeping your scheduled appointments. In the event you have failed to cancel 3 scheduled appointments you will be terminated from our practice.


To request a refill, please call your pharmacy, and allow 48 hours for processing. Please be aware that, depending on your medical condition, physician might request to see you to provide the refills. Please note that new prescriptions and antibiotics will not be prescribed over phone. Also, narcotic medication will not be prescribed over the phone or outside the normal working hours.

Release of Protected Health Information

The information contained in your medical record is strictly confidential. We value the trust you place in us to keep this information confidential and will not release information without appropriate authorization signed by the patient or legal representative. You can download our Release of Protected Health Information Authorization form and bring it to our office.
